"Diabetes Outreach Services is an innovative program that incorporates many healthcare providers and systems to manage elderly patients with diabetes. The Hartford Center of Geriatric Nursing Excellence at the University of Utah will provide [me] with the tools needed to appropriately research and evaluate other home-based diabetes programs in the United States."
